This section contains official OLCA forms for condominium owners. If you need a board or maintenance action, or wish to apply for the board, find the appropriate form below. Each form explains its purpose and submission procedure. Forms are offered in both MS Word and PDF. The MS Word version allows you to type into the form itself. The PDF version must be printed. Owner Exception Request Form
This form should be used when owners are asking for permission to do something outside of the normal condo rules. For example, owners may place pots, flags, statues, and other decorative items on their patio, balcony or sidewalks and garden areas per our condo rules. Limited in-garden planting by owners around buildings and the complex, or other exceptions to our condo rules, are allowed only after receiving written approval and evaluation.
Key Release Form
This form gives permission to the OLCA property manager to enter an owner’s unit so that a service person has access.
Condo Problem Submission Form
This form should be submitted when owners notice a landscape or building problem, or a problem around your building or our complex. Examples include, but not limited to, issues with any of the following:
- Tree, bush, lawn, garden
- Irrigation, heads, sprinklers
- Building trim, bricks, soffit, bib
- Driveway, sidewalk, cobble rocks, street
- Roof, gutter, vents
Window Replacement Request Form
This form is to allow Unit Owners to request permission to replace windows with the approval of the Building and Grounds Committee and Board of Directors to ensure that the new windows are acceptable.
ACH Authorization Form
This form is to allow Unit Owners to make condo association fees through ACH transfer.
